The Policy Scan Project, by the Institute for Global Tobacco Control (at Johns Hopkins University) tracks and reports regulatory approaches to e-cigarettes around the world. In 2012 and 2013 there was a flurry of tobacco company investment in e-cigarettes, both in the United Kingdom (UK) and in the United States (US), a trend which continued in subsequent years. In 2018, 8% of Pierce County 12th graders said they had smoked a cigarette in the past 30 days. Of the participants who were not using e-cigarettes, only 5.8% had quit smoking altogether by the end of the survey, while 9.9% had stopped smoking daily. The success of all these enterprises hinges on the claim that e-cigarettes are healthier than traditional cigarettes. Companies like to paint a black-and-white picture of a new era of safe smoking. "Cigarettes, you've met your match," NJOY proudly proclaims in its Super Bowl ads. E-cigarettes and vaping are part of a trend going back at least nine years in the U.S. First publicized as a safer alternative to smoking tobacco, vaping caught on because it didn’t contain the carcinogens or tars found in most smoking tobacco products. Also, vaping was supposed to eliminate the dangers of secondhand smoke to those nearby.
Television advertisements for cigarettes have been banned in the U.S. since 1971, but in the past few years supposedly healthier, battery-powered alternatives have landed numerous prime-time appearances. Electronic cigarettes, or e-cigs, as they are known, soaked up the spotlight in recent Super Bowl commercials, on late-night talk shows and in a comedy sketch during the 2014 Golden Globe Awards. Indeed, a recent survey shows that nearly 60 percent of Americans are now familiar with the sleek, smokeless devices.

Among adolescents who both vaped and smoked traditional cigarettes, a higher proportion in the This is Quitting program, about 51%, reported not vaping nor smoking at seven months, compared with 30% of the assessment-only group. Susceptibility to smoking or non-combustible nicotine use is also influenced by national tobacco control policies. In Europe, Finland has been among the countries with most advanced tobacco control regulations [31]. Still, there is a large difference in smoking and snus use between students in academically and non-academically oriented education [7]. For snus, Finland follows the EU-wide sales ban but cross-border traveller imports from Sweden are permitted and an illicit market exists [32]. For e-cigarettes, comprehensive regulations were introduced in 2016 including retail sale licensing and ban for distance sales, display and advertising at point-of-sale, other flavours than tobacco and use of e-cigarettes in smoke-free places [33].
Among people who were currently using e‑cigarettes, the proportion of people who used them daily remained stable from 2016 (42%) to 2019 (42%), then increased in 2022–2023 (49%). While more males than females used e‑cigarettes daily, the change among females was much greater than among males, rising from 36% in 2019 to 51% in 2022–2023. This corresponds to a fourfold increase in the number of females using e‑cigarettes daily, from approximately 70,000 to 300,000 (Figure 2).

"When you are out and about with your children and see an advertisement, for example, take the opportunity to talk about it," she says. As they grow older, parents can expand on their thoughts and expectations. "It’s also important to give teens and young adults the space to ask questions," she says. The 2023 NYTS showed that about 2 million middle and high school students reported using e-cigarettes in the last 30 days. Another study that year found that the rates of youth who say they vaped with nicotine in the past month more than doubled in two years. About 11% of high school seniors reported this habit in 2017, compared to 25%, or one in four seniors, in 2019.

Teens cannot participate in such studies, but "we all agree that e-cigs are not a good thing for youth and nonsmokers," Dr. Baldassarri says. Njoy’s products accounted for less than 3% of U.S. e-cigarette sales in the past year, according to retail data from Nielsen. Vuse, owned by Reynolds American, and Juul control about 60% of the market, while hundreds of disposable brands account for the rest. The market share of disposable e-cigarette products increased from 52.0% to 57.8% between 2022 and 2023. Among disposable e-cigarette products, those with flavors other than tobacco accounted for 94.7% of the market in 2023. Research indicates that e-cigarettes may emit harmful substances, which means there could be health risks to the user and others through secondhand exposure. There are also reports of lung illnesses and deaths related to inhalation of certain vaping oils into the lungs, which have no way to filter out toxic ingredients. You might be tempted to turn to electronic cigarettes (e-cigarettes, vape pens, and other nondisposable and disposable vaping devices) as a way to ease the transition from traditional cigarettes to not smoking at all. But is smoking e-cigarettes (also called vaping) better for you than using tobacco products? One chemical, known as 6-methyl nicotine, has been shown in rodent experiments to be far more potent than nicotine in targeting the brain’s nicotine receptors and more toxic than nicotine. Smoking harms nearly every organ in the body ― heart and blood vessel disease; lung disease; cancer (almost anywhere in your body); and impacts to your bones, eyes, teeth, gums, fertility and pregnancy. The latest research, published in a July National Center for Health Statistics data brief, shows that in 2021, e-cigarette use was highest among adults 18 to 24 years old.
Manufacturers claim e-cigarettes are a safe alternative to smoking regular cigarettes. Cancer is definitely a concern, given that vaping introduces a host of chemicals into the lungs. But vaping products haven’t been around long enough for us to learn whether or not they cause cancer. In 2018, 3.6 million U.S. high school and middle school students smoked an e-cigarette within 30 days of polling, making it the most common tobacco product used among this group.
The cells are also less able to form new vascular tubes and to migrate and participate in wound healing. With a 399.73% increase in retail e-cigarette sales (excluding internet sales and tobacco-specialty stores) from 2015 through 2020, the environmental consequences of e-cigarette waste are enormous. The fourth-generation vaporizers can also be customized and come with different types of heating coils — some intended for vaporizing solids, not liquids. In general, people using e‑cigarettes did not report doing so in order to quit smoking regular tobacco cigarettes. In 2022–2023, only 1 in 5 (21%) people who had ever used e‑cigarettes reported that they first used e‑cigarettes to help them quit smoking (Figure 5). The most common reason people gave for using e‑cigarettes was out of curiosity (58%).
To date, no e-cigarette product has been approved by the FDA for smoking cessation. In contrast, nicotine replacement therapies (e.g., patch, gum, lozenge) and some prescription medications (i.e., varenicline, bupropion) have been evaluated by the FDA and found to be safe and effective at helping people quit smoking. Manufacturers and sellers of e-cigarettes aggressively target young people.
It can also reduce fertility, increase the chance of developing type 2 diabetes, and contribute to bone loss. Even people who "smoke but don’t inhale" face an increased risk of mouth cancer. Although there is no federal excise tax on e-cigarettes, states have the authority to tax e-cigarettes. Some people believe e-cigarettes may help lower nicotine cravings in those who are trying to quit smoking. However, e-cigarettes are not an FDA-approved quit aid, and there is no conclusive scientific evidence on the effectiveness of vaping for long-term smoking cessation. It should be noted that there are seven FDA-approved quit aids that are proven safe and can be effective when used as directed.
"More research about the health effects of alternative tobacco products is critically needed to put science behind the regulation of the tobacco industry." As the debate blazes, deep-pocketed big tobacco investors are buying up e-cig companies, injecting millions of dollars into the market and banking on a bright future for the devices. More than 100 e-cigarette companies are now jockeying for the business of smokers and nonsmokers alike.
To our knowledge, no studies in Europe have examined susceptibility to e-cigarette (S-EC) use and snus use (S-SN). Glyoxal and methylglyoxal are formed by thermal degradation or oxidation of PG and VG87. Glyoxal is considered mutagenic, while the related compound methylglyoxal has been identified as a metabolite during glycolysis and is thus naturally present in the body. Methylglyoxal is also present in foods and drinks such as honey and coffee.
Most have a battery, heating element, and place to hold a liquid, usually containing highly addictive nicotine, that is added or included in the device. The heating element aerosolizes the liquid for the inhalation of the liquid nicotine or other substance. E-cigarettes are called many different names, including "vapes," "e-cigs," "vapor products," and "electronic nicotine delivery systems (ENDS)." The chemicals in electronic cigarettes, commonly called vapes, can severely damage the lungs, according to studies reviewed by the American Lung Association. The nicotine in them, which can be in higher quantities than traditional cigarettes, can also impact a child’s brain development. Several peer-reviewed studies found that nicotine negatively affects teens’ memory and attention and can increase their risk of developing other mental and behavioral problems later in life.
Although e-cigarettes do not give off smoke like tobacco cigarettes, they do expose people to secondhand aerosol or "vapor" that may contain harmful substances. Scientists are still learning about the health effects of being exposed to secondhand e-cigarette aerosol. The FDA monitors the national usage rates for all tobacco products, including an annual youth survey, and has seen a drastic increase in youth use of e-cigarette products in recent years.

Health experts are concerned about the various chemical ingredients within e-cigarettes. Research now shows that young people who initially use e-cigarettes, who have never smoked before, are 3 times more likely to take up tobacco smoking.
